Placebo gait retraining for use in knee osteoarthritis clinical trials does not change surrogate knee load measures: A randomized pilot study
نویسندگان
چکیده
Introduction: Gait retraining is a novel management strategy which has shown ability to reduce pain and the peak knee adduction moment (KAM) (a surrogate measure of medial load) in people with osteoarthritis. However, no published study compared gait placebo change does not affect KAM). Therefore, it unknown whether improvement due KAM reduction. Our primary aim was determine joint loading outcomes during placebo, toe-in, toe-out baseline. Secondary aims were evaluate effects three conditions on other lower limb trunk biomechanical outcomes. Methods: We conducted randomised repeated measures pilot following CONSORT extension for feasibility studies. Three-dimensional analysis performed five healthy female participants known pathology baseline gait, retraining. The involved walking more upright trunk. hypothesised altering sagittal plane position would impact load. Knee averaged over four-six trials included: early late stance KAM, impulse, flexion (KFM). Means 95% confidence intervals (CI) obtained using linear mixed models. Results: Mean CI (Nm/(BW*Ht)%) were: baseline: 4.78 (3.95, 5.59), placebo: 4.76 (3.93, 5.58); 3.74 (2.20, 5.29), 3.86 (2.31, 5.40). There difference KFM or impulse between placebo. Toe-in reduced from Placebo mean angle conditions. increased foot progression baseline, respectively. Discussion: load while achieved From perspective, we have developed promising condition clinical trials. Future studies may consider replicating this larger sample, group osteoarthritis, assessing long-term symptomatic effects, evaluating credibility participant perspective. Impact application field: can be used high quality controlled investigating effect symptoms Placebo-controlled will ascertain symptom reduction others factors such walking, clinician-patient interaction. Predicting Gait Retraining Strategies for Knee Osteoarthritis
Symptomatic knee osteoarthritis is one of the most common types of osteoarthritis and is one of the top ten causes of years of life lost due to disability. One common treatment strategy involves the prolonged use of anti-inflammatory medications. Recently, gait retraining has been proposed as non-invasive and non-pharmacological treatment option for knee osteoarthritis. However, many possible g...
متن کاملCumulative Knee Adductor Load and Clinical Measures in Knee Osteoarthritis
INTRODUCTION Knee osteoarthritis (OA) is a multi-factorial degenerative joint disaese resulting in pain and altered neuromuscular performance. Repetition and abnormal loading, combined with mechanical factors are attributed to the progression of knee OA [1,2], but the effects of repetitive weight bearing activity remain unclear. The cumulative knee adductor load (CKAL) is a parameter that repre...
متن کاملExercise Alters Gait Pattern but Not Knee Load in Patients with Knee Osteoarthritis
Six female patients with bilateral medial knee OA and 6 healthy controls were recruited. Patients with knee OA received a 6-week physiotherapist-supervised and home-based exercise program. Outcome measures, including the Western Ontario and McMaster Universities Arthritis Index and Short Form-36 Health Survey as well as objective biomechanical indices were obtained at baseline and follow-up. Af...
متن کاملEffect of ultrasound therapy for knee osteoarthritis: a meta-analysis of randomized, double-blind, placebo-controlled clinical trials
Objective: To perform a meta-analysis of randomized, double-blind, placebo-controlled clinical trials to separately assess the effect of continuous ultrasound (US) therapy and pulsed ultrasound therapy with low frequency for decreasing pain and improving physical function on people with knee osteoarthritis. Data sources: A systematic review (to April 2015) is conducted without language limits i...
متن کاملBromelain as an adjunctive treatment for moderate-to-severe osteoarthritis of the knee: a randomized placebo-controlled pilot study.
BACKGROUND Osteoarthritis (OA) of the knee is the most prevalent joint disorder. Previous studies suggest that bromelain, a pineapple extract, may be a safer alternative/adjunctive treatment for knee OA than current conventional treatment. AIM To assess the efficacy of bromelain in treating OA of the knee. DESIGN Randomized, double-blind placebo-controlled trial. METHODS Subjects (n = 47)...
